Job summary

Care Lync is hiring a Support Coordinator to help adults with disabilities and the aging population navigate services and secure resources for independence. You will assess needs, develop ISP plans, and advocate for participants while coordinating with healthcare professionals and families to deliver high-quality, person-centered care.

The role emphasizes travel to participant homes, professional development, and internal growth opportunities within a collaborative team.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify participant needs and goals.
• Develop, implement, and update Individual Support Plans (ISP).
• Provide participants with choice and advocacy in care options.
• Coordinate with medical professionals, families, and community resources.
• Monitor services to ensure safety, satisfaction, and progress toward independence.
• Support adults with disabilities and aging population in accessing community resources.
• Maintain documentation and ensure confidentiality and compliance.
• Participate in ongoing training and professional development.
