Job summary

Care Lync in Maine is seeking a dedicated Support Coordinator / Case Manager to help adults with disabilities and aging clients access housing, medical care, and community resources.

You will conduct person-centered assessments, develop and update Individual Support Plans, coordinate with providers, and advocate for participants to ensure safety, progress, and independence. Travel up to 50% to meet clients in their homes and communities.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify each participant's needs, preferences, and life goals.
• Develop, implement, and regularly update Individual Support Plans (ISP) to ensure participants receive the right services.
• Provide participants with choice and advocacy, helping them make informed decisions about providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver seamless, high-quality care.
• Monitor and evaluate services to ensure participants' safety, satisfaction, and progress toward independence.
• Support both individuals with disabilities and members of the aging population in accessing the community resources they need.
• Maintain documentation, confidentiality, and compliance with all program and ethical standards.
• Participate in ongoing training and professional development to strengthen your skills and expand your impact.
