Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort traveling up to 50% to meet participants in homes and communities.

Responsibilities

• Conduct person-centered assessments to identify participants' needs, preferences, and goals.
• Develop, implement, and regularly update ISP plans to ensure appropriate services.
• Provide participants with choice and advocacy in providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources.
• Monitor services for safety, satisfaction, and progress toward independence.
• Support adults with disabilities and aging populations in accessing community resources.
• Maintain documentation, confidentiality, and program/ethical compliance.
• Participate in ongoing training and professional development.
