Zusammenfassung

Care Lync in Pennsylvania is seeking a dedicated Support Coordinator to develop individualized care plans and coordinate services for adults with IDD, Autism, and physical disabilities enrolled in the Medicaid Waiver program. You will work to help participants live independently in their homes and communities.

This role requires travel up to 50% to participant locations, collaboration with the care team, and ongoing monitoring of outcomes while maintaining confidentiality and regulatory

Qualifikationen

• BS/BA degree in related field or equivalent.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and state-related Clearances.

Aufgaben

• Complete assessments to identify participant needs and desires for service delivery.
• Develops a Person-Centered Service Plan and update annually or as changes occur.
• Coordinate with providers and care teams to ensure ISP alignment and seamless service delivery.
• Monitor clinical outcomes and confidentiality of participant information.
• Travel up to 50% to participant locations as needed.
