Job summary

Care Lync is seeking a dedicated Support Coordinator / Service Coordinator to develop and manage individualized care plans for adults with intellectual and developmental disabilities across Pennsylvania. The role involves coordinating services, monitoring outcomes, and advocating for participant needs to promote independent living in home and community settings.

Responsibilities include assessments, ISP development, provider coordination, incident reporting, and maintaining confidentiality.

Qualifications

• BS/BA degree in related field.
• Minimum of 3 years case management experience for individuals with IDs/ASD/physical disabilities.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and updated state clearances.
• Valid driver’s license with personal vehicle insurance and current registration.

Responsibilities

• Complete assessments to identify participant needs.
• Develop a Person-Centered Service Plan and update annually.
• Coordinate with providers to ensure informed provider choices.
• Ensure Behavioral and Crisis Plans align with the ISP.
• Report incidents of abuse, neglect, or exploitation.
• Travel to participants up to 50% as needed.
• Maintain confidentiality of health information.
• Attend trainings and recertification classes.
