Job summary

Care Lync in Pennsylvania seeks a dedicated Support Coordinator to develop Individual Support Plans, coordinate services, and monitor outcomes to help adults with IDD and autism live independently.

Responsibilities include assessing needs, arranging housing, medical and community supports, and ensuring confidentiality while working with a multidisciplinary team. Travel up to 50% as needed.

Qualifications

• BS/BA degree in related field or coursework.
• Experience with Microsoft Office; familiarity with state-maintained databases.
• Maintain CPR/First Aid certification and updated state-related Clearances.
• Valid Driver's License; vehicle insurance and registration current.

Responsibilities

• Complete assessments to determine participant needs and service desires.
• Develop a Person-Centered Service Plan and update annually or on significant changes.
• Ensure participant chooses providers with information for informed decisions.
• Coordinate with other care providers to ensure seamless service delivery.
• Maintain confidentiality of sensitive health information; attend trainings.
• 50% travel to participant sites as needed.
