Job summary

Care Lync is seeking a Service Coordinator / Support Coordinator to develop individualized care plans and coordinate services for adults with intellectual, developmental, and physical disabilities in Pennsylvania. You will broker services, monitor outcomes, and ensure ISP alignment with behavioral and crisis plans.

The role requires strong documentation, confidentiality, and the ability to travel as needed to support participants in community settings.

Qualifications

BS/BA degree in related field.
Experience with Microsoft Office and state-maintained databases.
Ability to perform basic math using whole numbers, fractions, and decimals.
Maintain proficiency in company training and certifications.
Clean criminal background history.
Maintain state-related clearances.
Valid Driver's License with personal vehicle insurance and current registration.
Inform supervisor immediately about changes in insurance/policy or driver's license.

Responsibilities

Complete assessments to identify participant needs and desires for service delivery.
Develop and modify a Person-Centered Service Plan annually or with changes.
Coordinate with providers to ensure ISP aligns with Behavioral/Crisis plans.
Maintain documentation, reporting, and confidentiality.
Travel up to 50% to visit participants.
