Job summary

Care Lync seeks a dedicated Support Coordinator to provide case management for adults with IDD, autism, and physical disabilities enrolled in the Medicaid Waiver program. You will develop individualized care plans and coordinate services.

Travel up to 50% is required. The role monitors outcomes, advocates for housing, medical, and community supports, and ensures confidentiality. A BS/BA and MS Office proficiency are required.

Qualifications

• BS/BA degree required
• Proficiency with Microsoft Office and state-maintained databases
• Maintain CPR/First Aid certification and updated state related Clearances

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develops a Person-Centered Service Plan and updates at least annually.
• Ensure participant choices of providers with full information to make informed decisions.
• Coordinate with other care coordinators to ensure seamless delivery of services.
• Report incidents of abuse, neglect, or exploitation to supervisor immediately.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive information.
• Attend trainings and recertification classes.
• Communicate with supervisor about participant needs and limitations.
• Travel up to 50% as needed.
