Job summary

Care Lync is seeking a Support Coordinator / Service Coordinator / Case Manager to develop individualized care plans, broker services, and monitor outcomes for adults with IDD, autism, or physical disabilities enrolled in Medicaid waivers.

The role involves coordinating housing, medical, social, and community services, travel to participants, and ensuring confidentiality. Strong communication and organization are essential.

Qualifications

• BS/BA degree in related coursework or field.
• Minimum of 3 years' case management experience.
• Experience with Microsoft Office and state databases.
• CPR/First Aid certification.
• Valid driver's license with personal vehicle insurance.

Responsibilities

• Complete assessments to identify participant needs and service desires.
• Develop and annually update a Person-Centered Service Plan.
• Provide information to help participants choose providers.
• Ensure ISP aligns with Behavioral and Crisis Plans.
• Report abuse, neglect or exploitation to supervisor immediately.
• Coordinate with other care coordinators for seamless service delivery.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive health information.
• Attend required trainings and recertifications.
• Travel to participants as needed (up to 50%).
