Job summary

Care Lync is seeking a Support Coordinator to manage case work for adults with disabilities enrolled in Medicaid Waiver programs. You will create individualized care plans, broker services, and monitor outcomes to promote independent living in home and community settings.

The role requires coordinating with multiple providers, ensuring informed client choices, and traveling up to 50% to support housing, medical, social, and vocational needs.

Qualifications

• BS/BA degree or equivalent in a related field.
• Experience using Microsoft Office and state databases.
• Maintain CPR/First Aid certification and state-related clearances.

Responsibilities

• Conduct assessments to determine participant needs and service desires.
• Develop and update Person-Centered Service Plans annually or as needs change.
• Coordinate with providers to ensure informed choices and consistent service delivery.
• Monitor and evaluate services to ensure quality and cost-effectiveness.
• Travel to participant locations up to 50% to arrange housing, medical, social, and vocational supports.
