Job summary

Care Lync is seeking a dedicated Support Coordinator to manage casework for adults with disabilities, coordinating services and developing individualized care plans. You will play a key role in connecting people with housing, medical, social, and community supports to help them live independently.

The role requires a Bachelor’s degree or equivalent, CPR/First Aid certification, valid driver’s license, and ability to travel as needed.

Qualifications

• Bachelor-level degree required or equivalent experience in related field.
• CPR/First Aid certification and ongoing trainings are required.
• Must maintain proficiency in company sponsored training and certifications.

Responsibilities

• Conduct assessments to identify participant needs and desired services.
• Develops a Person-Centered Service Plan and updates it annually or with significant changes.
• Coordinate with providers to ensure informed choices and seamless service delivery.
• Maintain confidentiality of sensitive information and participant records.
• Travel up to 50% to participant locations as needed.
