Job summary

Care Lync is seeking a dedicated Support Coordinator to develop individualized care plans, coordinate services, and monitor outcomes for adults with IDD and autism in Pennsylvania. You will broker housing, medical, and community services within the ISP framework, ensuring compliant and confidential service delivery.

Flexibility includes travel up to 50% and a strong emphasis on client-centered planning, collaboration with care teams, and ongoing monitoring of clinical outcomes.

Qualifications

• BS/BA degree in related course work.
• Proficiency with Microsoft Office and state-maintained databases.
• CPR/First Aid certification and updated state clearances.

Responsibilities

• Complete assessments to identify participant needs and preferences for service delivery.
• Develop Person-Centered Service Plans and update annually or with major changes.
• Coordinate and broker housing, medical, social, vocational services within the ISP framework.
• Ensure Behavioral Support Plans and Crisis Plans align with the ISP and protect participant safety.
• Maintain confidentiality and handle sensitive health information per policy.
• Attend required trainings and recertifications; communicate effectively with supervisors.
