Job summary

Care Lync is seeking a Support/Service Coordinator to develop and implement individualized care plans for adults with IDD, Autism, and physical disabilities. The role focuses on brokering services, monitoring outcomes, and coordinating housing, medical, social, and community supports.

Responsibilities include assessing participant needs, creating ISP-based plans, and collaborating with providers to ensure cost-effective, comprehensive care. Occasional travel is required to support client needs.

Qualifications

• BS/BA degree required or related field.
• Minimum of 3 years of case management experience with ID/Autism/Physical Disabilities.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and state clearances.
• Keep trainings and certifications up to date as required by program regulations.
• Valid driver’s license with personal vehicle insurance and current registration.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop Person-Centered Service Plans and update annually or on significant changes.
• Provide information to help participants choose providers for fully informed decisions.
• Ensure Behavioral Support and Crisis Plans align with ISP requirements.
• Coordinate with other care professionals to ensure seamless service delivery.
• Monitor and evaluate services to ensure professional, comprehensive care.
• Travel up to 50% to meet participants as needed.
