Job summary

Care Lync is seeking a Service Coordinator / Support Coordinator / Case Manager to manage individualized care plans and coordinate services for adults with disabilities across Pennsylvania. You will broker and monitor services, develop ISP-based plans, and ensure outcomes are achieved in a professional, cost-effective manner.

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office and state-maintained databases.
• Clean criminal background history.
• CPR/First Aid certification maintained.
• Valid Driver's License and current vehicle insurance/registration.
• Maintain training certifications as required by program regulations.

Responsibilities

• Complete assessments to identify each participant's needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and update annually or with changes.
• Ensure participant choice of providers and ISP alignment with Behavioral/Crises Plans.
• Maintain accurate documentation, reporting, and coordination with other care entities.
• Travel to participant homes and communities up to 50% of the time.
