Job summary

Care Lync is a social services agency focused on Support Coordination for adults with IDD, autism, and other disabilities enrolled in Medicaid Waiver programs. We provide comprehensive case management, service brokerage, and ISP development to help individuals live independently in home and community settings.

The role involves assessing participant needs, creating person-centered plans, coordinating with providers, and ensuring quality outcomes while maintaining confidentiality.

Qualifications

• BS/BA degree in related field or equivalent experience.
• Experience with Microsoft Office and state-maintained databases.
• CPR/First Aid certification maintained and up to date.
• Valid Driver's License with active auto insurance and registration.

Responsibilities

• Conduct assessments to identify participant needs and service desires.
• Develop person-centered service plans and update as needed.
• Explain provider options to participants for informed decisions.
• Coordinate with care teams to ensure integrated service delivery.
• Maintain participant confidentiality and comply with regulations.
• Travel to participant locations as required (up to 50%).
