Job summary

Care Lync is a social services agency delivering Support Coordination services to adults with Intellectual, Developmental, and Physical Disabilities enrolled in Medicaid Waiver programs. The role focuses on developing individualized care plans, coordinating services, and monitoring outcomes to support independent living in home and community.

The position requires a BS/BA degree and at least 3 years in case management, with travel up to 50%.

Qualifications

• BS/BA degree in related course work.
• Minimum of 3 years related experience providing case management.
• Experience with Microsoft Office and state-maintained databases.
• Maintain proficiency in company sponsored training and certifications.
• Maintain CPR/First Aid certification and updated state-related Clearances.
• Complete and maintain required training as per program regulations.
• Valid driver’s license with personal vehicle insurance and current registration.

Responsibilities

• Complete assessments to identify participant needs and desires for services.
• Develop Person-Centered Service Plans and update annually or with significant changes.
• Ensure informed provider choices and alignment with ISP.
• Coordinate with other care coordinators for integrated service delivery.
• Monitor and adjust services to ensure quality, confidentiality, and cost-effectiveness.
• Travel to participants up to 50% as needed.
