Care Lync is seeking a dedicated Support Coordinator/Service Coordinator/Case Manager to develop individualized care plans and coordinate services for adults with IDD, Autism, and disabilities under Medicaid Waiver programs. The role emphasizes administering ISP-based care, coordinating with providers, and monitoring outcomes to ensure cost-effective, comprehensive support.

Qualifications

BS/BA degree in related course work.
Experience with Microsoft Office, state-maintained databases.
Maintain valid driver’s license and personal vehicle insurance coverage; have vehicle registration on file with Care Lync.
Inform supervisor immediately when any change in auto insurance, policy, or license status occurs.

Responsibilities

Complete assessments to identify participant needs and desires for service delivery.
Develop a Person-Centered Service Plan and update annually or with significant changes.
Assist participants in selecting providers to ensure informed decisions.
Coordinate with care team to ensure seamless delivery of services.
Coordinate and monitor services and supports, including housing, medical, social, vocational, and other community services.
Travel up to 50% to participant locations as needed.
