Job summary

Care Lync is a Social Services agency focused on providing Support Coordination services to adults with Intellectual, Developmental and Physical Disabilities and Autism Spectrum Disorder enrolled in Medicaid Waiver programs. Our team helps clients navigate and access services to live independently in their home and community.

The Support Coordinator / Service Coordinator / Case Manager develops individualized care plans, coordinates services, and monitors clinical outcomes to ensure professional

Qualifications

• BS/BA degree in related course work.
• Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
• Experience with Microsoft Office, state-maintained databases.
• Must maintain CPR/First Aid certification and updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.
• Inform supervisor immediately when any change in auto insurance/policy or license status.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develops a Person-Centered Service Plan and update annually or after major changes.
• Provide information to help participants choose providers and make informed decisions.
• Ensure Behavioral/Crisis Plans align with the ISP.
• Report all incidents of abuse, neglect, or exploitation to supervisor promptly.
• Coordinate with other care coordinators to ensure seamless care delivery.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive health information.
• Attend required trainings and recertification.
• Consult with supervisor to consider participants’ needs, abilities, and limitations.
• Travel up to 50% to participants as needed.
