Job summary

Care Lync is seeking a dedicated Support Coordinator / Service Coordinator / Case Manager to join our team. You will develop individualized care plans, coordinate services, and monitor outcomes to help adults with IDD and autism live independently in their home and community.

Travel up to 50% to meet participants, maintain confidentiality, and collaborate with supervisors and other coordinators to connect housing, medical, social, vocational and other supports.

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office and state-maintained databases.
• CPR/First Aid certification and updated state clearances.
• Valid driver’s license including personal vehicle insurance coverage.

Responsibilities

• Complete assessments to identify participant needs for service delivery.
• Develop a Person-Centered Service Plan and modify at least annually or with major changes.
• Ensure participant provider choices and informed decisions.
• Coordinate with other care coordinators to ensure seamless service delivery.
• Ensure confidentiality of sensitive information.
• Attend required trainings and recertifications.
• Travel up to 50% to participants as needed.

Education

BS/BA degree in related course work

CPR/First Aid certification

Maintains state-related clearances
