Job summary

Care Lync, a social services agency, is seeking a Support Coordinator / Service Coordinator to develop and implement individualized care plans for adults with IDD and disabilities under the Medicaid Waiver program. You will broker, coordinate, and monitor services to ensure cost-effective, quality outcomes in home and community settings.

The role includes helping clients access housing, medical and social supports, communicating needs to the care team, and traveling to participants as needed to

Qualifications

BS/BA degree in related course work.
Experience with Microsoft Office and state-maintained databases.
Maintain state related Clearances and complete required trainings.
Commitment to ongoing training and certifications as required by program regulations.
Valid driver’s license and vehicle insurance coverage.

Responsibilities

Complete assessments to identify participant needs and desires for service delivery.
Develop a Person-Centered Service Plan and update it at least annually or with major changes.
Assist participants in selecting providers by giving information for informed decisions.
Coordinate with providers and other care coordinators to ensure seamless service delivery.
Ensure confidentiality of sensitive information and compliance with policies.
Travel to participants as needed up to 50%.
