Care Lync is a Social Services agency focused on providing Support Coordination (Service Coordination/Case Management) services to adults diagnosed with Intellectual, Developmental and Physical Disabilities and Autism Spectrum Disorder who are enrolled in the Department of Human Services, Medicaid Waiver Program.

Our Support Coordinators help clients navigate and access services to live independently in their home and community in Maine.

Formación

• BS/BA degree in related field required.
• Minimum of 3 years related experience providing case management.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and up-to-date state clearances.
• Valid Driver's License with personal vehicle insurance and current registration.

Responsabilidades

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and modify annually or with changes.
• Ensure participant choice of providers and informed decision making.
• Ensure Behavioral/ Crisis plans align with the ISP where applicable.
• Coordinate with other care coordinators to ensure seamless services.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive health information.
• Attend required trainings and recertifications.
• Travel up to 50% to participant homes as needed.
