Job summary

Care Lync is a Social Services agency focused on providing Support Coordination to adults with intellectual, developmental, and physical disabilities and autism who are enrolled in Medicaid Waiver programs. We help individuals live independently by coordinating services and developing individualized care plans.

The Support Coordinator develops Person-Centered Service Plans, coordinates with providers, monitors outcomes, and ensures confidentiality.

Qualifications

• BS/BA degree in related field.
• Experience with Microsoft Office and state-maintained databases.
• Maintain state related Clearances and trainings as required.

Responsibilities

• Complete assessments to identify participant needs.
• Develop Person-Centered Service Plans and modify annually or with changes.
• Coordinate with providers to ensure ISP alignment with Behavioral/Crisis Plans.
• Ensure confidentiality of sensitive information and documents.
• Travel up to 50% to participants as needed.
