Job summary

Care Lync is seeking a dedicated Support Coordinator to manage individualized care plans for adults with intellectual and developmental disabilities and autism. You will coordinate services, monitor outcomes, and advocate for clients to help them live independently in their home and community.

The role requires developing ISP plans, coordinating with providers, and ensuring confidentiality, with up to 50% travel to meet participants’ needs.

Qualifications

• BS/BA degree in related field.
• Minimum of 3 years experience in case management for individuals with Disabilities.
• CPR/First Aid certification and updated state clearances.
• Valid driver's license and vehicle insurance coverage, current vehicle registration, on file with Care Lync.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop and modify a Person-Centered Service Plan for each participant and update annually or with significant changes.
• Coordinate with providers to ensure ISP alignment with Behavioral Support Plan and Crisis Plan if applicable.
• Ensure participant confidentiality and professionalism in care delivery.
• Actively coordinate with other care coordinators and entities to ensure seamless service delivery.
• Travel up to 50% to participant sites.
