Job summary

GT Independence, LLC is seeking a Support Broker to coordinate daily Medicaid Waiver services and promote the growth and independence of participants. You will work to maintain quality documentation and collaborate with staff, providers, and families to ensure program success.

Responsibilities include monitoring health and safety, assisting with spending plans, and guiding participants in accessing community supports. Travel to events and meetings is required.

Qualifications

• Bachelor-level or higher with relevant population experience and background in social services.
• Master's degree or RN/related credential required or preferred depending on role.
• Criminal background checks compliant with CA and federal requirements.

Responsibilities

• Coordinate daily operation of Medicaid Waiver services.
• Promote independence and growth for participants.
• Maintain documentation in compliance with state and agency requirements.
• Develop relationships with agency staff, providers, and families.
• Deliver training and support to staff; monitor quality and safety.
