Job summary

GT Independence, LLC is seeking a Support Broker to coordinate the daily operation of Medicaid Waiver services and promote independence for participants under state and program regulations. The role emphasizes collaboration with GT staff, family members, and community partners to ensure high-quality, person-centered supports.

You will build relationships, maintain documentation, and participate in performance improvement to safeguard health, safety, and service quality.

Qualifications

• Master's degree or equivalent in a relevant field and experience with the population.
• Bachelor's degree with 2 years of experience or RN/AS with 3 years.
• Strong interpersonal, communication, and documentation skills.

Responsibilities

• Coordinate daily operation of Medicaid Waiver services and support participants.
• Build relationships with staff, providers, and families to promote independence.
• Ensure accurate documentation per HCBS and program requirements.
• Travel to events, conferences, and training as needed.
• Provide instruction and counseling to guide problem-solving and decision-making.
• Support participants in budgeting, ISP outcomes, and community integration.
