Job summary

You will develop relationships with participants, families, providers, and community resources, ensure compliant documentation, and provide advocacy and decision-making support.

Responsibilities include service planning, resource coordination, education on self-directed services, and collaboration with healthcare providers to meet participant needs.

Qualifications

• DHCF waiver requirements knowledge or willingness to learn.
• Criminal background check clearance as per applicable laws.
• Experience with Medicaid waiver programs and service coordination.

Responsibilities

• Develop and maintain relationships with participants, families, providers, and community resources.
• Ensure timely, accurate documentation and compliance with agency and state requirements.
• Provide guidance, advocacy, problem-solving, and decision-making support.
• Assist with service planning, resource coordination, and access to community supports.
• Educate participants and families on self-directed services and employer responsibilities.
• Coordinate with healthcare providers to support participant needs.
• Assist participants with housing and independent living resources.
