Job summary

GT Independence in San Francisco is seeking a Support Broker to coordinate daily Medicaid Waiver services and promote participant independence in line with state regulations. You will collaborate with GT staff to ensure timely documentation, engage participants, and support meaningful community connections.

The role emphasizes building relationships with participants, families, and providers, driving quality and compliance, and contributing to the GT mission through service excellence.

Qualifications

• Be 18 years of age or older.
• Have current, appropriate licensure.
• State-specific criminal background checks completed.
• No concurrent employment with VF/EA FMS-Support Broker entity.
• Excellent interpersonal skills.

Responsibilities

• Promote growth, independence, and quality of life for participants through support and staff training.
• Build and maintain relationships with staff, service providers, and families.
• Ensure documentation meets state and agency requirements including HCBS waiver.

Education

Master’s degree in social work, Psychology, Counseling, Rehabilitation, Nursing Gerontology or Sociology and one year experience with the population(s).

Bachelor’s degree and two years’ experience with the population(s).

Registered Nurse (RN) or associate degree with three years’ experience with the population(s).
