GT Independence is seeking a qualified Support Broker to coordinate Medicaid Waiver services and promote participant growth and independence. The position requires meeting DHCF waiver case manager requirements, including appropriate licensure or degrees, plus relevant experience.

The Support Broker is responsible for coordinating the daily operation of Medicaid Waiver services, and for promoting personal growth and independence of participants in accordance with state and program regulations, GT Independence Policy and Procedures, and applicable accreditation standards.
