Job summary

GT Independence is seeking a qualified Support Broker to coordinate Medicaid Waiver services and promote participant growth and independence. The role focuses on building relationships with participants, families, providers, and community resources while ensuring documentation and compliance with state requirements.

The position requires meeting DHCF waiver case manager requirements, including appropriate licensure or degrees, plus relevant experience.

Qualifications

• Meet the DHCF requirements for a waiver case manager.
• Have a master's degree in social work, Psychology, Counseling, Rehabilitation, Nursing Gerontology or Sociology and one year experience; OR bachelor's degree and two years’ experience.
• RN qualification accepted.
• Associate degree plus three years’ experience accepted.

Responsibilities

• Develop and maintain relationships with participants, families, providers, and community resources.
• Ensure timely, accurate documentation and compliance with agency and state requirements.
• Provide guidance, advocacy, problem-solving, and decision-making support.
• Assist with service planning, resource coordination, and access to community supports.
• Educate participants and families on self-directed services, budgeting, employer responsibilities, and staff management.
• Coordinate with healthcare providers and community organizations to support participant needs.
• Assist participants with housing and other independent living resources.
