Qualifications

• Excellent interpersonal and written communication abilities.
• Ability to manage a schedule and travel as needed.
• Familiarity with state waiver programs and documentation requirements.

Responsibilities

• Coordinate daily operation of Medicaid Waiver services.
• Promote growth and independence of participants.
• Maintain accurate documentation and weekly scorecards.
• Collaborate with staff and external stakeholders.
• Assist with onboarding and community resource connections.
