Job summary

GT Independence, LLC is seeking a Support Broker to coordinate daily Medicaid Waiver services and promote participants’ growth, independence, and community inclusion. You will work with participants, families, and providers to ensure quality, compliant services and timely documentation while upholding GT’s mission and values.

This role requires excellent interpersonal skills, ability to travel within California, and a professional background in social work or related fields.

Qualifications

• Be 18 years of age or older.
• Master’s degree in social work or related field and one year experience with the population; OR a Bachelor’s with two years’ experience; OR RN with relevant experience.
• Criminal background checks as required by CA and federal law.

Responsibilities

• Coordinate daily Medicaid Waiver services and promote participants' growth and independence.
• Build relationships with participants, families, staff, and providers to ensure quality services.
• Ensure documentation accuracy, timeliness, and compliance with state requirements.
• Travel within California to attend events, meetings, and community activities.
• Educate and support participants in developing social networks and community connections.
