Responsibilities

• Promote personal growth, independence, and quality of life for service participants through support and monitoring.
• Build and maintain positive relationships with agency staff, service providers, and individuals/families.
• Ensure accuracy and timeliness of documentation in compliance with state requirements.
• Provide training and guidance to support staff and participants in problem-solving and decision-making.
• Coordinate services with various stakeholders to promote self-determination.
