Qualifications

• District of Columbia Qualified Practitioners of CCC are licensed professionals: physicians, psychologists, LICSWs, LCSWs, APRNs, RNs, LISWs, LPCs, PAs, LMFT, LGPCs.
• Performance expectations include clinical care coordination encounters, crisis intervention, referrals to services, documentation in encounter notes, and treatment planning updates.
• Preference will be given to applicants with experience working with DC residents with SUD/mental health comorbidity and with a treatment-team environment.

Responsibilities

• Coordinate clinical care across SUD Level 1 Outpatient and Level 2.5 Day Treatment.
• Ensure referrals, authorizations, and transitions to appropriate levels of care.
• Conduct follow-up and ensure linkage to other services and community resources.
• Maintain encounter documentation and participate in discharge planning and transfers.
