Job summary

Apex Recovery LLC (TN) is seeking a certified or registered SUD Counselor to support clients requiring significant care coordination and referrals. You will link clients with recovery resources, provide information and follow-up, and participate in treatment planning alongside the clinical team.

The role involves intakes, group facilitation, and maintaining accurate client records, while collaborating with community agencies to support client progress toward goals.

Qualifications

• Certified or Registered SUD Counselor in TN preferred.
• Bachelor's Degree in counseling, psychology, or social work preferred.
• At least 2 years of clinical experience, preferably with co-occurring disorders.
• Ethics and standards relevant to client/staff interactions in therapeutic/case management environments.

Responsibilities

• Coordinate care for clients with high-need care coordination and referrals.
• Link clients to resources relevant to support recovery.
• Provide information, support, and follow-up services as needed.
• Participate in treatment team meetings and contribute to treatment plans and diagnoses.
• Develop and maintain a current database of relevant resources for program clients.
• Complete client intakes as necessary.
• Facilitate SUD groups as needed.
• Maintain client records per quality improvement guidelines and contract requirements.
• Coordinate with other community, health, and welfare agencies.
• Support program progress toward goals as part of the team.
