Job summary

Qualifications

• Knowledge of care for individuals receiving MOUD or related treatment.
• Ability to establish referral systems with community agencies.
• Strong communication with diverse health and social groups.
• Proficiency in English, both oral and written.

Responsibilities

• Coordinate substance use, behavioral health, and general health services for clients.
• Manage a caseload as assigned by supervisor.
• Liaise between psychiatry, social work, and community groups to ensure seamless care.
• Ensure follow-up treatment and medical care for patients.
