Job summary

Creative Pathways Inc. seeks a Clinical Care Coordinator (CCC) to oversee substance use disorder program operations in DC. You will coordinate client referrals, authorizations, transitions to levels of care, and follow-up services while ensuring compliance and effective team collaboration.

The role requires a clinical degree, DC licensure, and 2+ years of care coordination experience in SUD/mental health, with strong EMR skills and travel within the district.

Qualifications

• Must possess an active DC license as a Qualified Physician, Psychologist, Nurse RN, Clinical or Intermediate licensed Social Worker LGSW, LICSW, Professional Counselor LPC, or Marriage and Family Therapist.
• Two years of experience providing care coordination, counseling, or rehabilitation services in a SUD or mental health setting with co-occurring disorders.
• Valid DC driver’s license, proof of auto insurance, and ability to travel within the district using personal vehicle.

Responsibilities

• Develop and regularly review individualized treatment/service plans with clients to implement appropriate services and develop effective alliance with patients.
• Provide ongoing supportive and clinical coordination for group and individual appointments per service plan.
• Collaborate with client to access services, facilitate transitions of care, arrange housing, transportation, and address social determinants of health.
• Coordinate Interdisciplinary Team Meetings with client and family members when applicable.
• Work with primary counselor, case manager and referrals to develop outcomes-focused, person-centered service plans.
• Maintain close communication with the primary counselor for input on program compliance and engagement.
• Provide or facilitate home or community-based visits as needed.
• Identify and address obstacles to high-quality care and client satisfaction.
• Demonstrate ethical behavior and treat clients with integrity.
