Job summary

Telecare Corporation is seeking a Substance Use Counselor Case Manager to support individuals with mental illness and addictions in a direct care role at an on-site facility in San Diego, CA. The position involves managing client care, coordinating services, and promoting independent living within the community.

The role requires CADC/CATC or SUDCC certifications or WA CDP equivalents, with at least 6 months’ experience in addictions.

Qualifications

• Minimum 6 months of experience in an addictions setting.
• High School Diploma or GED with CADC/CATC or SUDCC/SUDCC II/III/IV, or WA CDP certification.
• Valid driver’s license and insurance; willingness to use personal vehicle for clients.

Responsibilities

• Serve as primary support and case manager for individuals with mental illness focusing on substance abuse.
• Assist clients to live as independently as possible in the community.
• Coordinate scheduling, transportation and billing as needed.
