Telecare Corporation is seeking a Case Manager Substance Use Counselor Certified to support individuals with mental illness, focusing on substance abuse and building independence in the community. The role is full-time with daytime shifts (8:30 AM - 5:00 PM, Monday to Friday) in California, with competitive starting wages based on geography and experience.

• Must have High School diploma or GED and CADC/CATC/SUDCC/CDP (or equivalent) certification.
• Six months of experience in addictions setting.
• Certifications must be maintained for duration of employment.
• CPR, First Aid, and CPI certifications required by date of employment.

Responsabilidades

• Serve as primary support and case manager for individuals with mental illness, focusing on substance abuse and addictions.
• Assist clients to live as independently as possible in their chosen community.
• Coordinate services, crisis intervention, and linkage to housing, transportation, and benefits.
