Job summary

Telecare Corporation is seeking a Case Manager Substance Use Counselor to support adults with mental illness and substance use needs in the Oakland area. You will act as the primary case manager, helping clients access services and live more independently in their communities.

The role requires CADC/CATC/SUDCC/SUDCC II/III/IV or equivalent licensure, a minimum of six months’ experience, CPR/First Aid and CPI certifications, and a valid driver’s license.

Qualifications

• High School Diploma or GED and relevant certification(s) for Substance Use Counseling.
• Six (6) months’ experience in addictions setting.
• Certifications must be maintained for duration of employment.
• Willingness to use personal vehicle for client transport and learn billing codes.

Responsibilities

• Serve as the primary case manager for individuals with mental illness focusing on substance use treatment.
• Assist clients to live as independently as possible in their chosen community.
• Coordinate services and treatment plans, including billing codes within two months.
