AppleOne Employment Services is seeking an Outreach Specialist for a Pasadena-based nonprofit dedicated to helping individuals experiencing homelessness. The role focuses on street outreach, housing navigation, and case management to connect clients with housing, benefits, healthcare, transportation, and long-term stability.

Qualifications

• Minimum 1 year of experience working with individuals experiencing homelessness, or 2 years of experience in social services, behavioral health, substance use services, or case management.
• Experience using HMIS documentation or similar case management systems.
• Strong communication, relationship-building, and client engagement skills.
• Strong organizational skills with the ability to manage documentation, follow-up tasks, and competing client needs.
• Proficiency with Microsoft Office and data entry systems.
• Valid California driver's license and reliable transportation required.

Responsabilités

• Conduct street outreach and engage individuals experiencing homelessness with professionalism, empathy, and consistency.
• Assess, screen, and triage client needs, referrals, and eligibility for housing and supportive services.
• Assist clients with obtaining required documentation for housing programs, benefits, and community resources.
• Connect clients to healthcare, mental health services, substance use treatment, employment support, public benefits, and transportation assistance.
• Support housing placement efforts, including permanent supportive housing and affordable housing programs.
• Maintain accurate case notes, HMIS documentation, and data entry while participating in case conferencing, team meetings, trainings, and community collaboration.
