Job summary

Union Station Homeless Services is seeking a Peer Outreach Specialist in Altadena, CA to conduct outreach and housing navigation for chronically homeless individuals. You will collaborate with a case manager and firefighters within PORT to engage clients in housing pathways and address immediate needs.

Responsibilities include referrals, document collection for housing applications, data entry in HMIS, and linking clients to mental health, health, and social services, with transportation support

Qualifications

• Outreach and housing navigation for chronically homeless individuals.
• Knowledge of HUD guidelines and subsidized housing programs.
• Data entry into HMIS and basic office software proficiency.
• Strong communication, collaboration and problem-solving skills.

Responsibilities

• Conduct outreach to engage chronically homeless individuals toward permanent housing.
• Receive and triage referrals from partners and organizations.
• Assist with collecting documents for permanent supportive housing applications.
• Coordinate services with other providers and attend meetings.
• Link clients to mental health, health, and social services as needed.
• Provide transportation assistance to stabilize housing crises.
• Document services and maintain HMIS records.
