Avenue 360 Health and Wellness is seeking an Outreach Navigator/Assessor to identify, assess, and connect individuals experiencing homelessness with housing resources via the Coordinated Entry System. The role conducts outreach, HMIS data entry, and helps secure required documentation for housing programs.

The position emphasizes community outreach, client engagement, and collaboration with a multidisciplinary team, with training in HMIS and Coordinated Entry procedures to support PSH and RRH

Qualificações

• Bachelor's degree in social work, human services, public health, or related field; equivalent experience.
• Minimum two years of experience in social services, housing programs, homelessness outreach, or case management.
• Experience working with vulnerable populations including individuals experiencing homelessness.
• Strong ability in client engagement, outreach, and housing navigation.
• Data entry and documentation in HMIS or similar systems.
• Strong communication, problem-solving, and interpersonal skills.

Responsabilidades

• Conduct outreach and complete client enrollments and housing assessments through the Coordinated Entry System (CES).
• Provide pre-navigation services to assist with eligibility documentation for PSH and RRH.
• Dedicate a minimum of four hours per week to community outreach activities.
• Provide transportation and logistical support for housing-related activities (ID documents, SSN cards, disability verification, etc.).
• Attend required Housing Assessor, Housing Navigator, and HMIS trainings.
