Job summary

Insight Housing in Albany, CA is seeking a Housing Navigator to perform street outreach, housing navigation, and case management for residents experiencing homelessness. This field-based role requires engagement on outreach routes and coordination with CES assessments.

You will build housing stability plans, assist with housing search and placement, and support participants through transition to permanent housing, including coordination with emergency shelter and transition activities through

Qualifications

• High school diploma required; Bachelor’s degree in Social Services or related field preferred.
• Proficient in computer use; Microsoft Office (Word, Excel, Outlook, Teams).
• Willingness to perform field-based work outdoors for extended periods.
• CPR/First Aid training within last five years preferred.
• Valid California driver license and access to a personal vehicle.

Responsibilities

• Conduct field-based street outreach along assigned routes to engage unsheltered residents.
• Build trusting relationships and perform housing navigation and case management.
• Complete Coordinated Entry System (CES) assessments for participants who consent to services.
• Develop and update housing stability plans every 90 days for engaged participants.
• Support housing search, placement, and retention, including income and identification documents.
