Job summary

Avenue 360 Health and Wellness is seeking an Outreach Navigator/Assessor to identify and connect individuals experiencing homelessness with housing resources via CES. You will conduct outreach, HMIS-based assessments, and provide pre-navigation services to help clients obtain documentation for PSH or RRH.

The role requires at least two years in social services, strong client engagement, and the ability to work independently and in teams.

Qualifications

• Bachelor’s degree or equivalent experience in social services or related field.
• Valid driver’s license preferred for outreach and transportation duties.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Conduct outreach and complete client enrollments and housing assessments through the Coordinated Entry System (CES).
• Provide pre-navigation services to obtain eligibility documentation for PSH and RRH.
• Dedicate a minimum of four (4) hours per week to community outreach activities, including community calls and encampment reconnaissance.
• Provide transportation and logistical support to assist clients with housing-related activities.
• Attend required Housing Assessor, Housing Navigator, and HMIS trainings.
