Job summary

Avenue 360 Health and Wellness seeks an Outreach Navigator/Assessor to identify and connect homeless individuals with housing resources through the CES. You will conduct outreach, complete housing assessments in HMIS, and help clients obtain documentation for housing programs.

The role emphasizes community engagement, transportation support, and participation in required trainings to ensure successful placement in PSH or RRH, aligning with our values and mission.

Qualifications

• Bachelor's degree or equivalent experience in social services, housing, or public health.
• Minimum two years in social services, housing programs, homelessness outreach, or case management.
• Strong client engagement, outreach, and housing navigation skills.
• Experience with HMIS or similar case management systems and data entry.
• Valid driver’s license preferred for outreach and transportation duties.

Responsibilities

• Conduct outreach and complete client enrollments and housing assessments through CES.
• Provide pre-navigation services to obtain eligibility documentation for PSH and RRH.
• Dedicate at least four hours weekly to community outreach activities and events.
• Provide transportation and logistical support for housing-related activities and moves.
• Attend Housing Assessor, Housing Navigator, and HMIS trainings as required.
