Job summary

Union-Station-Homeless-Services is seeking an Outreach Specialist in Pomona, CA to conduct outreach and housing navigation for individuals experiencing homelessness. You will assist with document gathering for housing, connect clients to supportive services, and link them to permanent housing opportunities while maintaining HMIS records.

The role requires effective communication, case management skills, and the ability to multi-task in a dynamic environment.

Qualifications

• Bachelor's degree preferred.
• 1 year of experience working with homeless individuals in outreach/navigation or case management.
• Successful demonstration of required skills through prior USHS employment may be considered in lieu of years of experience.

Responsibilities

• Conduct outreach to engage individuals experiencing homelessness and support pursuit of permanent housing.
• Receive, assess and triage referrals from partners.
• Conduct standardized screening and triage.
• Provide services and interventions using best practices to meet participant needs.
• Assist with collecting documents for permanent supportive housing applications.
• Connect to supportive services (mental health, medical care, substance abuse, employment) as needed.
• Provide linkage to mainstream resources (DPSS, Social Security Disability).
• Provide transportation assistance.
• Assist accessing permanent housing opportunities (PSH, move-in help, shared housing, affordable housing).
• Conduct follow-up to ensure housing plan completion.
• Document services and data in HMIS within required timeframes.
• Attend case conferences and trainings as required.
