Job summary

Northshore Home Health Care is seeking a licensed Speech-Language Pathologist to deliver in-home and senior living community assessments and therapy for older adults, using a flexible, patient-centered approach to maximize safety and independence.

You will collaborate with a multidisciplinary team, develop individualized care plans, document in EMR, and utilize telehealth to expand access. The role offers flexible schedules and opportunities for ongoing education and career growth.

Qualifications

  • Must have a valid state licensure or be eligible for SLP licensure.
  • Degree from an accredited Speech-Language Pathology program.
  • Proficiency with EMR systems and basic computer skills.
  • Current CPR certification.

Responsibilities

  • Deliver one-on-one speech therapy sessions in homes and senior living communities.
  • Develop individualized care plans and implement evidence-based treatments.
  • Document clinical notes accurately in EMR systems.
  • Collaborate with PTs, OTs, and fitness staff.
  • Educate patients, caregivers, and staff on safety and aging-in-place strategies.
  • Utilize telehealth to increase access and flexibility.

Job description

Role Overview: A licensed Speech Language Pathologist (SLP) providing personalized assessment and therapy to older adults in private homes and Senior Living Communities, utilizing a flexible, patient-centered approach.

Key Responsibilities
  • Deliver one-on-one speech therapy sessions to enhance mobility, independence, and quality of life.
  • Conduct evaluations, develop individualized care plans, and implement evidence-based treatments.
  • Document clinical notes accurately within electronic medical records (EMR) systems.
  • Collaborate with interdisciplinary teams including PTs, OTs, and fitness specialists.
  • Educate patients, caregivers, and staff on safety and aging-in-place strategies.
  • Utilize telehealth and virtual care to increase access and flexibility.
Qualifications & Skills
  • Valid state licensure or eligibility in Speech Language Pathology.
  • Degree from an accredited program.
  • Proficiency with basic computer and EMR systems.
  • Current CPR certification.

Physical & Additional Requirements: Ability to perform moderate physical tasks, assist with patient transfers, and drive between locations. Must wear PPE as needed.
