Job summary

Personal Touch Home Health Care Services Inc. is seeking a licensed Speech Language Pathologist to provide in-home and community-based therapy for older adults, promoting mobility, independence, and quality of life.

You will evaluate, plan, and deliver evidence-based therapy, document care in EMR, and collaborate with interdisciplinary teams, including caregivers. Telehealth options are used to increase access and flexibility.

Qualifications

  • Valid state licensure or eligibility in Kansas.
  • Degree from an accredited Speech-Language Pathology program.
  • Proficiency with EMR systems and basic computer skills.
  • Current CPR certification and ASHA CCC.
  • Ability to meet physical demands including lifting and driving between locations.

Responsibilities

  • Conduct evaluations and develop individualized treatment plans.
  • Deliver evidence-based therapy to older adults in-home and in community settings.
  • Document care accurately in electronic medical records.
  • Collaborate with interdisciplinary teams including PTs, OTs, and caregivers.
  • Educate patients, families, and staff to enhance safety and aging-in-place outcomes.
  • Utilize telehealth and virtual care options to increase access.

Job description

Seeking a licensed Speech Language Pathologist to provide personalized, in-home and community-based care to older adults, promoting mobility, independence, and quality of life.


Key Responsibilities Include

  • Supporting patient wellness, functional independence, and mobility through therapy sessions.

  • Documenting care accurately using electronic medical records in compliance with regulations.

  • Collaborating with interdisciplinary teams including PTs, OTs, and caregivers.

  • Educating patients, families, and staff to enhance safety and aging-in-place outcomes.

  • Utilizing telehealth and virtual care options to increase flexibility and access.


Qualifications And Skills Include
