Job summary

Health Here is seeking a Speech Language Pathologist to provide personalized care to older adults in private residences and senior living communities. You will evaluate, design treatment plans, and deliver evidence-based therapy to improve wellness, mobility, and independence.

Responsibilities include documenting care in EMR, collaborating with an interdisciplinary team, educating patients and caregivers, and incorporating telehealth when appropriate.

Qualifications

• Valid state licensure or eligibility and degree from an accredited program.
• Proficiency with computers and EMR; CPR certification current.
• Ability to meet physical demands of in-home care, including lifting, repositioning, and driving.
