Job summary

Rock Health seeks a Speech Language Pathologist to provide personalized care to older adults in private homes and senior living communities. You will evaluate patients, create treatment plans, and deliver evidence-based therapy to support wellness, mobility, and independence.

The role requires licensing, an accredited degree, and CPR certification. You will document progress in EMR, educate patients and caregivers, and collaborate with an interdisciplinary team, with telehealth as an option when

Qualifications

• Valid SLP license or eligibility.
• Degree from an accredited program.
• Proficiency in computer and EMR systems.
• Current CPR certification.

Job description

Role:

Speech Language Pathologist providing personalized care to older adults in private homes and senior living communities.

Responsibilities:

Conduct evaluations, develop treatment plans, deliver evidence-based therapy to support patients' wellness, mobility, and independence. Document progress in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.

Qualifications:

Valid SLP license or eligibility, degree from an accredited program, proficiency in computer and EMR systems, current CPR certification.

Skills & Physical Requirements:

Ability to exert medium physical effort, assist with patient transfers, perform CPR, reposition patients, and drive to patient locations. Must meet physical demands of in-home care and wear PPE as needed.
